CIMIT and Institutional Forms

CIMIT FORMS

In addition to the forms below, please refer to proposal instructions and requirements for the specific grant in which you are applying.

  1. Budget Form: For all new proposals, the budget must be submitted along with a budget justification.

  2. Compliance Form: For all new proposals, the compliance form must be submitted with the application.

  3. No-Cost Extension Form: To request a no-cost extension for a current award, complete this form and email it to Lesley Watts at least ten days prior to the current period of performance end date.

INSTITUTION FORMS

For applications submitted by investigators from consortium institutions, institutional signature is required. Your institution’s standard format may be used or, if you prefer, your authorized institutional official can indicate approval by signing the budget page.

For new proposals submitted by investigators at Massachusetts General Hospital, the Partners Research Proposal Coversheet Form should be signed by the PI and Chief and submitted.

GRANTS & AWARDS

For CIMIT Innovation Grants and CIMIT Prize for Primary Healthcare, applications are submitted through the CIMIT web-based submission system, CoLab. Please go to the web page of the specific grant in which you are applying to view and download instructions on how to use CoLab.
