The forms listed below are required for most CIMIT Innovation Grant full proposal applications. When applying for a grant, please refer to the grant's specific proposal instructions and requirements.

Partners Research Proposal Coversheet Form (Revised: 12/1/09)
For new proposals submitted by investigators at MGH, this form should be signed by the PI and Chief and uploaded following the instructions in the submission site: https://precisionconference.com/~cimit.

For applications submitted by investigators from other institutions, institutional signature is required.  Your institution’s standard format may be used or if you prefer, your authorized institutional official can indicate approval by signing the budget page (see link below). 

Budget Form (Revised: 12/16/09)
For all new proposals, the budget must be uploaded along with a budget justification following the instructions in the submission site: https://precisionconference.com/~cimit.

Compliance Form
For all new proposals, the compliance form must be uploaded with the application following the instructions in the submission site: https://precisionconference.com/~cimit..

No-Cost-Extension Form (Revised: 11/23/09)
To request a no-cost-extension for an existing award for all projects that need an extension of the performance period, this form should be completed in full and emailed to Kirby Vosburgh at kvosburgh@partners.org.
